Complete manuscripts are due by September 21, 2009 (deadline extended).

Authors should submit the following BY EMAIL:

• Complete manuscripts in two file formats, Microsoft Word (.doc) and .pdf
• Appropriate Copyright Transfer Agreement, duly signed by authorized author, then scanned
• Photo of yourself in .jpg format (pre-sized to 1.25" wide by 1.75" tall)
• Extended Abstract (for printing in Proceedings Abstract Volume) in both .doc and .pdf format

Copyright Transfer Agreement

There are two copyright transfer agreement types; one for authors who hold the copyright to the work, and one for employers who hold the copyright to the work under the work-for-hire doctrine.  Please select the appropriate form based upon who owns the copyright to the work, and execute as instructed on your chosen form.

Form 1: I hold the copyright to this work.

Form 2:  My employer holds the copyright to this work.
